From the Bridge House ‘Path to Home’ website:

Path to Home – Interactive, Safe and Consistent Overnight Sheltering

Path to Home provides:

Walk-up sheltering coupled with support services  including intake and assessment, navigation, and direct financial assistance for basic needs.

There is no one size fits all solution to homelessness, but through this is new sheltering model we are able to help clients determine their own path toward housing.

Path to Home is open 7 days a week and offers case management services on site.

After the first night of sheltering those who wish to stay for more than one night will complete a “welcome meeting” – a motivational interview conducted by one of our case managers to determine each persons’ goals and needs.

Once the assessment is complete a clear step-by-step plan is put in place with an intended exit strategy. Daily progress is tracked by the program staff.

Breakfast is provided by Community Table Kitchen and all day bus passes are available to allow clients to engage in their service plans.
Path to Home will be open 7 days a week from 7:00 PM to 10:00 AM.
